Home Health Services to Support Healing at Home

For many hospital patients, being discharged doesn’t necessarily mean their healing and recovery process is over. Many times, they need to continue their rehabilitation at home. Without the support and resources of the hospital, it can be tough for some people to adhere to the instructions their doctor has given them. Caring Hearts® is here to ensure they get the care they need while recovering at home.

Many people are anxious to get back to normal once they leave the hospital, yet a recovery can be set back considerably if they do more than they are able or instructed to. We can bridge the gap from hospital to a full recovery with our in-home Medical Care. Caring Hearts offers licensed and trained professional Caregivers who have extensive experience providing home health care and medical services. They can help clients recover from ongoing conditions such as congestive heart failure, COPD, diabetes, heart disease, and cerebral vascular diseases.

Specialized Medical Care & Senior Home Health Care

If you have a loved one with medical needs that require the assistance, our Caregivers will be there for them when you can’t be. We understand that many of our clients are capable of living independently and simply need a helping hand on occasion. Caring Hearts is ready to step in and provide that assistance.

  • Our Caregivers offer specialized medical care including:
  • Catheter care
  • G-Tube feeders
  • Wound care
  • Lab venipuncture
  • First aid/CPR

Steps to Care

  1. 1.
    Initial Call Once we’ve received your information, we will have a short 5-10 minute call to answer any pressing concerns and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Care Plan customized to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
